- Download and complete Registration and Waiver Form by 5:00 p.m. on March 22 to be eligible to participate. Fax completed form to (916) 627-4203, or email to afrost@elkgrovecity.org, or drop off at City Hall (8401 Laguna Palms Way, Elk Grove – ATTN: Angela Frost)
